This letter is to request that you provide copies of the following public information in accordance with Connecticut General Statutes, CGS as amended and as well as relevant FOIC mandates and recent rulings.
1. One copy each of the digital recording of the SFD Board of Fire Commissioners monthly meeting of Monday, September 12, 2011
2. One copy each of any correspondence received or sent by SFD, electronic and/or hardcopy, relative to corrections made to the 2010 GL and referenced in tax collector memorandum to SFD chief, dated 8/11/2011. Also, please provide official property number for RO5356. ( ref. comments SFD meeting - property taken for non-payment)
3. One copy each of invoices associated with Fleet Services dated 7/23 & 8/23/2011 as noted in monthly expense detail for July and August of 2011 under 7401 - Fuel. Same for 7803 dated 8/05/2011 ESI Debit $1,225.08.
4. One copy of contract for current SFD annual audit services.
5. One copy of any electronic or hard copy correspondence, sent or received, relative to information provided the City of
6. One copy each of any correspondence sent to or received , electronic and /or hardcopy, from any employee of CVH, SFD and /or the City of Middletown relative to dispatch protocol/changes and/or the recent decision to place a SFD vehicle with staffing on the grounds of CVH.
I am assuming your usual positive cooperation by having copies of this simple request immediately upon receipt distributed to all SFD Commissioners. Also, I anticipate your timely response and also request that any written correspondence be via the following provided official contact address. Should you determine and I assume after consultation with other SFD Commissioners, that there may be any expense to me for providing said documents and/or access; I am requesting that you notify me in advance of such.
