You may access the MCA Grant forms from "grants" under the "resources" menu item on arts2go.org. Alternatively, you may also find the forms on the City website under the Arts & Culture subheading.
These forms are in PDF format and may be filled in by computer. Categories 2 and 3 are for project support in the amounts of $1 up to $8,000.
